We will make every effort to qualify your financing request the day we receive it. Please fill in the application completely and sign at the bottom so we can do our part to make sure your equipment financing request gets processed quickly and smoothly.
Step #2: Personal Financial Statement Personal Financial Statement (PFS) Complete - Print - Sign - Fax to 612-435-4811 We are currently asking all Applicants to complete the Personal Financial Statement (PFS) when applying for financing. Step #3: Documentation Needed Please fax the following documentation to 612-435-4811 - Tax Returns (Personal & Business) - last 2 years - Bank Statements (Business) - last 3 months - Balance Sheet / Profit & Loss Statement (Business) - Equipment Description / Sales Invoice / Vendor Contact - Judgments or Collections? - fax proof of 'paid' status. - Bankruptcy? - fax a copy of the Discharge Papers. *Publicly traded corporations will need to provide the last 2 Annual Reports with the applicable SEC Form 10-K reports. **You can email all documents to Info@AlphaEquipmentLeasing.com Credit Report Information: AnnualCreditReport.com is a centralized service for consumers to request free annual credit reports. It was created by the three nationwide consumer credit reporting companies - Equifax, Experian and TransUnion. AnnualCreditReport.com provides consumers with the secure means to request and obtain a free credit report once every 12 months from each of the three nationwide consumer credit reporting companies in accordance with the Fair and Accurate Credit Transactions Act (FACT Act). AnnualCreditReport.com offers consumers a fast and convenient way to request, view and print their credit reports in a secure Internet environment. They also provide options to request reports by telephone and by mail. AnnualCreditReport.com is the only service authorized by Equifax, Experian and TransUnion for this purpose. The three nationwide consumer credit reporting companies have always encouraged consumers to regularly review their credit reports. AnnualCreditReport.com will not approach consumers via email, telemarketing or direct mail solicitations. Please note that, as a security precaution, Consumers should never provide their personal information to any other company or person for requesting free annual credit reports under the FACT Act.
